In treating partial seizures, the medication can be used on its own or along with other medications. The difference is only found at the high-end of dosing:

• The initial adult dose of Trileptal is 300 mg twice a day (600 mg total).
• After at least one week, dosage may be increased to 600 mg twice per day (1200 mg total).
• After at least another week, dosage may be increased again to 1200 mg twice per day (2400 mg total).
• It is rare for dosage to exceed 600 mg twice per day (1200 mg total) when other medications are being taken; most patients cannot tolerate so much of the medication.
• As a monotherapy, higher doses of 1200 mg twice per day (2400 mg total) are fairly normal.

In patients with impaired renal function, initiate Trileptal at one half the usual starting dose and increase slowly to achieve the desired clinical response.

Patients who buy Trileptal to treat bipolar mania should be aware that the medication works about as well as lithium. If already taking lithium or some other medication that is producing good results, there's no reason to switch to Trileptal.

Those who aren't having good results from lithium or other medications---either they're not working or they can't be taken due to allergies---may want to try Trileptal. Treatment typically consists of 600 mg twice per day (1200 mg total) for adult patients. Younger patients will, of course, require adjusted doses.

In both cases the medication should continue to be taken for as long as it provides benefit.
